School Health Requirements

Preschool:

-Physical Exam Form-Each student should have a physical exam filed with the school by six weeks after the first day of school. By law, students will not be able to attend until this is completed.

-Immunization or Exemption Certificate-Each student should have a certificate displaying that they have had at least one of each required immunization series OR a notarized medical or religious exemption certificate. By law, students will not be able to attend until this is completed.

Kindergarten:

-Immunization or Exemption Certificate-Each student should have a certificate displaying that they have had at least one of each required immunization series OR a notarized medical or religious exemption certificate. By law, students will not be able to attend until this is completed.

-Dental Screening-Must be completed no earlier than age 3 and no later than 4 months after enrollment. Can be completed by the school nurse.

-Blood Lead Level

-Vision Screening-Must be completed no earlier than 1 year prior to enrollment and no more than 6 months after enrollment. Can be completed by the school nurse.

3rdGrade:

-Vision Screening-Must be completed no earlier than 1 year prior to enrollment and no more than 6 months after enrollment. Can be completed by the school nurse.

7thGrade:

-TDaP Vaccination-Must have a one-time dose of Tdap prior to 7thgrade entry.

-Meningococcal Vaccination-Must have one dose of meningococcal conjugate vaccine (A,C,W,Y). There is no provisional period to implement this requirement.

9thGrade:

-Dental Screening-Each student entering 9thgrade should have a dental certificate on file. The screening must occur no earlier than 1 year before enrollment and no later than 4 months after enrollment. Screenings can only be performed by dentists or dental hygienists.

12thGrade:

-Meningococcal Vaccination-Must have two doses of meningococcal conjugate (A,C,W,Y); or 1 dose if one dose was previously received when the student was sixteen years of age or older. There is no provisional period to implement this requirement if the student hasn’t received a single dose.
